Horizon Home Health Agency can provide a specific service or a coordination of health care services all in the comfort of your home. Under the direction of your physician our Director of Nursing will establish a Plan Of Care based on your needs and goals. Our professional services include:

"Quality Care In The Comfort Of Home"

The Skilled Nurse is an RN or LVN with training and experience in providing care in the home. The nurse communicates frequently with your physician to ensure that your care is current and effective. The licensed and experience nurses will perform a range of duties, including but not limited to medication managment, teaching and instruction on different diagnoses and disease process. Assessing you or your loved one for signs or symptoms that need to be addressed and reported to the physician.

Supplies/Therapy/Equipment: Medical Supplies, Durable Medical Equipment (DME): These devices may be required to complete your plan of care. While recieving services we are happy to coordinate the purchase of medical supplies for our patients. DME (e.g., walker, wheelchair, hospital bed) is covered separately and may be supplied by the home health agency or an outside Medicare-certified supplier of your choice.

Physical, Occupational and Speech Therapy services are provided by a licensed therapist or licensed therapy assistant under the direction of the therapist. Your therapist will provide specific information about the services and treatments you will receive.

The Medical Social Worker provides short-term counseling services,
referral to and coordination with community resources and assistance with living arrangements, finances and long-range planning.

Home Health Aide services are delivered under the supervision of a nurse. Our aides have training and experience in home care.
Typical duties include bathing, shampooing hair, changing bed linen,light housekeeping, meal preparation and
assistance with other activities of daily living. Additional duties
may be assigned by the nurse based on the thorough assesment of you or your loved one.

At Horizon Home Health Agency we understand that your medications can be overwhelming our skilled nurses will administer, teach and/or set up mediciations for you based on your needs and the care plan.
